INFORMED CONSENT,
ASSUMPTION OF RISK
& RELEASE OF LIABILITY

This agreement must be read in full and signed before any coaching services begin. Each section requires your acknowledgment before submission is permitted.

I
The Parties
Identify yourself and confirm the agreement parties

This Informed Consent, Assumption of Risk, and Release of Liability Agreement ("Agreement") is entered into between the undersigned individual ("Client") and FORGE14, operated under the name FORGE14 at forge14.vip ("Coach").

This Agreement governs all coaching services provided by FORGE14, including but not limited to: strength and conditioning programming, nutrition guidance, biweekly performance reviews, Biological Age Score assessments, and any associated digital or written communications.

II
Nature of Coaching Services
Understanding what FORGE14 provides and does not provide

FORGE14 provides online fitness coaching services, including custom training program design, nutrition programming, biweekly performance analysis, and the FORGE14 Biological Age Score metric. All services are delivered remotely via digital communication, email, and written program documents.

FORGE14 does not and cannot: supervise Client in person during training sessions; monitor Client's physical environment or equipment condition; provide medical advice, medical diagnoses, or medical treatment; serve as a substitute for a licensed physician, physical therapist, dietitian, or mental health professional; or guarantee specific fitness or health outcomes.

The FORGE14 Biological Age Score (BAS) is a proprietary coaching metric for tracking functional performance trends — it is not a clinical measurement, medical biomarker, or diagnostic tool. It is not intended to diagnose, treat, cure, or prevent any health condition.

Health & Medical Disclosure
Your representations regarding your physical condition

Client represents and warrants to FORGE14 that, to the best of Client's knowledge:

(a) Client is physically capable of engaging in exercise and physical training activity; (b) Client has disclosed, or will disclose prior to the start of any program, all known medical conditions, injuries, physical limitations, medications, and other health factors that may affect Client's ability to safely participate in a fitness program; (c) Client has either obtained, or if required by Client's medical condition, will obtain, physician clearance prior to beginning the program; and (d) Client understands that concealing or misrepresenting health information may result in programming that is unsafe for Client's condition and that FORGE14 assumes no liability for injuries arising from information Client failed to disclose.

Client agrees to immediately notify FORGE14 of any change in health status, new injury, medical diagnosis, or change in prescription medications during the coaching relationship.

Assumption of Risk
Acknowledgment of inherent risks associated with physical training

Client acknowledges that physical exercise and fitness training involve inherent risks of injury, illness, and in extreme cases, death, which may occur even when proper safety protocols are followed. Client voluntarily assumes all such inherent risks.

Client specifically acknowledges the following non-exhaustive list of risks associated with participation in a fitness coaching program:

  • Muscle strains, pulls, and tears
  • Joint sprains, dislocations, and ligament damage
  • Bone fractures and stress fractures
  • Back, spine, and neck injuries
  • Cardiovascular events including heart attack
  • Stroke and cerebrovascular events
  • Dehydration, heat exhaustion, and heat stroke
  • Hypoglycemia and metabolic events
  • Rhabdomyolysis from overexertion
  • Equipment-related injuries
  • Exacerbation of pre-existing conditions
  • Delayed-onset muscle soreness (DOMS)
  • Overtraining syndrome and burnout
  • Nutritional deficiencies from dietary changes
  • Allergic reactions to recommended supplements
  • Psychological stress related to program demands

Client further acknowledges that because coaching services are delivered remotely and online, FORGE14 has no control over Client's training environment, equipment condition, training surface, or the presence of other individuals in Client's training space. Client assumes full responsibility for ensuring the safety of their training environment.

Release of Liability
The core legal release — read this section with particular care

IN CONSIDERATION of being permitted to participate in FORGE14 coaching services, Client, on behalf of Client and Client's heirs, assigns, personal representatives, and next of kin, HEREBY RELEASES, WAIVES, DISCHARGES, AND COVENANTS NOT TO SUE FORGE14, its owner(s), operators, contractors, agents, representatives, and successors ("Released Parties") from any and all claims, demands, losses, liabilities, damages, injuries, costs, and expenses (including attorneys' fees) arising out of or in connection with Client's participation in FORGE14 coaching services.

This Release includes, but is not limited to, claims arising from:

(a) Ordinary negligence of Released Parties in the design, delivery, or administration of coaching programs;
(b) Client's failure to follow program instructions, safety guidelines, or coach recommendations;
(c) Client's pre-existing medical conditions that are aggravated or worsened during participation;
(d) Equipment failure or malfunction in Client's training environment, including equipment not owned or provided by FORGE14;
(e) Errors or omissions in nutrition programming, supplement recommendations, or performance tracking;
(f) Client's voluntary participation in exercises, loads, or intensities that Client chooses to exceed beyond what is prescribed;
(g) Any adverse health outcome arising from dietary changes recommended in the program.

LIMITATIONS: This Release does not apply to claims arising from gross negligence, willful misconduct, fraud, or intentional harm by Released Parties. Nothing in this Release shall be construed to limit claims that cannot be waived under applicable law.

Indemnification
Client's responsibility to protect FORGE14 from third-party claims

Client agrees to indemnify, defend, and hold harmless FORGE14 and its Released Parties from and against any and all claims, actions, damages, liability, and expenses (including reasonable attorneys' fees) brought by any third party arising out of or in connection with: (a) Client's participation in the coaching program; (b) Client's use of any exercises, nutritional advice, or recommendations provided by FORGE14; or (c) Client's breach of any representation, warranty, or obligation under this Agreement.

Emergency Medical Authorization & Contact
Authorization for emergency care and emergency contact information

In the event of a medical emergency arising during or in connection with the coaching program, Client authorizes FORGE14 to contact Client's designated emergency contact and, where FORGE14 has knowledge of an emergency, to direct Client to seek emergency medical services. Client acknowledges that FORGE14 operates remotely and that Client is solely responsible for calling emergency services (911 or local equivalent) in the event of an acute medical emergency during training.

Program Conduct & Client Compliance
Your responsibilities as a FORGE14 client

Client agrees to the following standards of conduct and compliance during the coaching relationship:

(a) Stop When in Pain. Client agrees to immediately cease any exercise that produces sharp, joint-based, radiating, or alarming pain, and to notify FORGE14 before continuing training. Muscle fatigue and appropriate soreness are expected; acute pain is not.

(b) Follow Prescribed Loads. Client agrees not to voluntarily exceed prescribed weights, repetitions, or intensities without coach authorization. FORGE14 assumes no liability for injuries arising from Client's self-imposed increases beyond the program.

(c) Maintain a Safe Environment. Client is responsible for ensuring their training space, equipment, and floor surface are safe before each session. FORGE14 is not responsible for equipment Client owns, borrows, or uses.

(d) Honest Reporting. Client agrees to provide accurate and honest information in all check-in forms and communications. FORGE14's programming decisions are based entirely on data Client provides; inaccurate reporting releases FORGE14 from any adverse outcomes arising from programming based on that data.

(e) Not a Medical Service. Client agrees not to use FORGE14's coaching program as a substitute for medical care. Client agrees to consult their physician regarding any medical symptoms, conditions, or concerns.

Legal Provisions
Governing law, severability, and full agreement

Governing Law. This Agreement shall be governed by and construed in accordance with the laws of the State of California, without regard to conflict of law principles. Any disputes arising under this Agreement shall be subject to the exclusive jurisdiction of the courts of Los Angeles County, California.

Severability. If any provision of this Agreement is found to be unenforceable or invalid under applicable law, that provision shall be limited or eliminated to the minimum extent necessary, and the remaining provisions shall continue in full force and effect.

Entire Agreement. This Agreement constitutes the entire understanding between Client and FORGE14 regarding liability, risk, and Client's consent to participate in coaching services. It supersedes all prior oral or written representations on these matters.

Electronic Signature. Client agrees that an electronic signature, digital signature, or typed acknowledgment on this form constitutes a legal, valid, and binding signature with the same legal force and effect as a handwritten signature, pursuant to the Electronic Signatures in Global and National Commerce Act (E-SIGN) and applicable California law.

Voluntary Agreement. Client certifies that they have read this Agreement in its entirety, have had the opportunity to ask questions, and are signing voluntarily without any duress, undue influence, or misrepresentation.
